STUDY ON WOUND DRESSING AND SELF-CARE BEHAVIOR OF DIABETIC PATIENTS IN TUVALU OUTPATIENT DEPARTMENT

Abstract

Purpose

In Tuvalu, obesity becomes more common due to increased westernization of eating habits and frequent celebration parties, registered in the capital PMH hospital diabetes 290 patients, hypertension 391, an average age of 56.6 years, an average body weight of 92.5 kg, an average BMI of 34.4, an average FBS of 10.7 mmol/L. Previous study found that the control of diabetes in non-communicable diseases (NCDs) patients was poor in Tuvalu. The plans to enhance patient compliance and treatment outcomes are assessed in this study because of the account local advantages such as population concentration, the self-control status of the local residents by monitoring the FBS, BP, BMI and other indicators of chronic diseases such as DM, hypertension and obesity are investigated in this study. Finally, the results of this study could be as the reference to reduce the incidence of DM, blood pressure and obesity to produce end-stage renal disease and other complications.

Method

From August 1 to November 30 in 2016, collected daily dressing’s output patients’ FBS, BP, and BMI.

Outpatient Dressing Results
From August 1 to November 30 in 2016, the records of outpatient dressing with a total of 73 patients were collected and 12 non-diabetic patients were among these patients. The mother island distribution of patients showed that the North Island was 43.8%, and the central Island was 30.1%. The average age was 50.4 years old and the average height was 168.7 cm. The average weight was 92.8 Kg. The number of patient with the weight more than 100 kg was 24 and 34.9% of all. Their average BMI was 32.4, and the average BP was 138.6 / 85.9 mmHg, the average FBS was 8.7.In this study, the result showed the average dressing 6.5 times for each patient; total dressing number was 1167 patients, and average of 16 patients a day.

Among the 73 patients, 12 had nondiabetic patients, with 6 males (50%) and 6 females. The average weight was 93.5 Kg, the average age was 43 years, the average height was 171.2 cm, the average BMI was 32.6, the average BP was 134.3 / 129.7 mmHg, the FBS was 6.4.
After four months of follow-up, in diabetic and nondiabetic patients as a whole, BMI, SBP, DBP, FBS, were not improved, indicating that patient compliance was still poor. Observed patients had 7-10 dressing changes. The answer of the remaining patients whether followed the rules of medication, a rule walking weight loss, control diet was no. Because local people's attitude was towards life, it is a big challenge for the public health preventive medicine angle promotion.

Conclusion and Suggestion
Obesity is the leading cause of cardiovascular disease, hypertension, diabetes, high blood cholesterol and other chronic diseases. Due to the country's food culture with high-calorie foods and sugary beverages have been occupied in every store and even elementary schools, and frequent civil celebrations. The problem of diet is very important and coupled with the hot weather which resulted in the residents don’t want to activities and other factors. It is recommended to apply the primary education of healthy diet and the impact of obesity. Moreover, the follow and record of the daily diet of patients with chronic diseases and content for assessing the patient's ability to move were recommended. To arrange the most suitable exercise for those who cannot move to the hospital and to strengthen health education home visits were necessary. Improving the quality of life of residents and prevention of serious complications and even cause death could reduce the burden on the economy.
